I have lost my certificate. How do I apply for a replacement?

There is a charge for this service of:

£12.00 per certificate or £7.50 per certificate if you are a member of the IoS - membership number required.

Replacement Certificates prior to 1995

£27.50 per certificate or £15.00 per certificate if you are a member of the IoS - membership number required

If you require a replacement please put your request in writing, providing us with any previous names and addresses plus approximate dates when you gained your qualification and include your date of birth.

Payment needs to be made at the time of application in the form of a cheque or postal order, made payable to 'Amateur Swimming Association'.

Please note that if your payment is by cheque then it will take approximately two weeks for clearance.
