Medical Declaration Forms

This form MUST be completed by anyone that is representing Great Britain or their Home Country internationally or competing in a British Swimming, ASA, SASA or WASA National event (all disciplines, excluding masters) and returned to the ASA Membership Department at the address below.  A new form MUST be completed annually even if the medication prescribed has not been altered or if no medication is being taken and whenever the medication is changed.

Skill Assessments 

Important information.

Revised skill levels are in force from 1 January 2010.

Skill level 1

Skill level 2

Skill level 3

Skill level 4

Skill level 5

From 1 January 2010, all advanced Assessors must attend a short review of skill levels 4 & 5 before attending an assessment day, due to the number of changes to the revised skill levels .

Please ensure all applications are submitted in bulk by clubs and not separately by individual swimmers. All clubs are to provide sufficient, appropriately qualified assessors to accompany their swimmers. 

0 - 3   swimmers        no assessor will be required
4 - 5   swimmers        1 assessor will be required, 
6 - 15 swimmers        2 assessors will be required
16-25 swimmers        3 assessors and this will increase in multiples of ten.

Important Note.

From 1 January 2009, following a fail at a skill assessment day, no Swimmer can retake their skill at the same level within 1 calendar month unless the fail on the day was on retakes.  This information must be shown in the Swimmers log book as 'failed on retakes'. Swimmers must produce their log books at each skills assessment.  If a Swimmer attends a skills assessment without their log book they will be refused entry.
